Show active diagnoses when adding new ones

Please add the ability to show active/current diagnoses when going in to chart notes to add ones. I have sometimes mistakenly added double diagnoses. Basically, remove options for diagnoses that have already been added or grey them out.

Tyson Edwards: If you want the diagnosis included in each note, you have to add it in. However, if you've previously diagnosed that patient with the same disorder, it adds an entry every time you see the patient and chart.

This patient, for example. Only 2 diagnoses should be showing up here. When I'm in a note I should be able to see previous diagnoses and just indicate that it's still active

To add on to this: The ability to delete a diagnosis within an unlocked note. I mistakenly double-added a diagnosis, and now it's in the current note twice with no ability to delete one of them.
