Display Name and Age At the Top of a Progress Note

If this could be displayed even when you scroll down to the bottom of the note that would be amazing! Too often I will end up with two notes open, and I have to scroll all the way back up to confirm which patient it is I am charting on.

Ok, circling back here.
There are 2 places you can select a 'legal name' for the client for insurance and non-insurance billing. If you did this and used the 'preferred name' in the regular client profile name fields, the correct name will show at the top.
Places to set the legal name:
- Billing Recipient popup from Client Profile --> Invoices Tab --> Ellipsis button --> Billing Recipient -- Client Legal Name.
- Client Claim Defaults popup from the Client Profile --> Claims tab --> Claim Defaults --> Patient Legal Name Field
So just to confirm, the ask here is to be able to see:
- The 'name they go by'
- The 'name they are billed to'
- DOB
- Age at the time the note was taken
- Age age they are today
Is this correct?
If so, as far as the name, we may can add some logic that says,
- If the 'legal name' is filled out in the Billing Recipient popup, display [Legal First Name] "[First name from Client Profile]" [Legal Last Name] on the Note.
- If the Patient Legal Name is filled out in the Client Claim Defaults, display [Legal First Name from Claim Defaults] "[First name from Client Profile]" [Legal Last Name from Claim Defaults] on the Note.
- If it's filled out in both, use the name in the Claim Defaults as the 'legal' name.

Tyson Edwards: Honestly, I voted for this because I am REALLY needing to see the patient's age when I am IN a note, preferably up near his name in the upper left corner so I don't have to leave the note, go to the profile, get the age, and then return to the note. Please, I beg of you, let the age show by the patient's name so it can be seen when we are in a note. :-)

Sorry, how are you currently defining the preferred name in the application currently?
Are you setting the name they go by as the First Name and then setting their legal name here for billing purposes?

Tyson Edwards: Since I don't bill insurance, I don't worry much about that setting unless they're asking for a superbill or something. I use a custom field on the overview page for identifying their preferred name, but because it's a custom field, it doesn't integrate nicely with other aspects of the system, such as searching by name.

Tyson Edwards: Yes, this is how I'm currently doing it, even though I also don't bill insurance but I do provide superbills so I need the "legal" name there.

Tyson Edwards: That's a good question and it does work, but it would still be very helpful to see the legal name and the "preferred name"/name the person goes by next to each other, as sometimes I need to know both for whatever reason. Here's an example of how Simple Practice does the name entry and then how it shows up on every page for the client's chart. And to clarify, I think DOB and age are both really helpful to see quickly next to the name.

Then as far as age, would you want age at the time the note was created/locked (for legal reasons) or the age as of today?

Tyson Edwards: Thanks, Tyson--that sounds great about the legal/defined name. In terms of the age, it should probably be the age at the time the note was created for legal reasons? Is it too much to ask for the age in the note in addition to today's age/DOB at the top of the record itself (next to the name at the top) so that's always visible when going through different tabs?

Tyson Edwards: I'd be happy to share my formatting. However the default names in the system are the names the person builds/uses every day. I then added a "Legal Name" custom field to the profile and mapped it. When the patients do their intake form that are asked for their name and their "Legal name" if different. Then in billing I set the billing name to legal since it's what insurance needs. I also build the legal name in my Rx system (something to keep in mind when you have eRx). Also keep in mind that some people have a name, a legal name and a different name with insurance since they may have things in different stages of completion.

I have learned over the years that it can be insulting to some people to say "preferred name." Just throwing that out there. It would be great to have this feature, but I wouldn't want a new feature to be accidentally insulting.
For example, my name is Hannah and I am a cisgender woman. Someone has never labeled Hannah my "preferred name" -- it's just my name. On the other hand, my transgender and non-binary clients tell me they get asked their "preferred name" often. It's insulting, it's just their NAME. Many of these clients have gone through the lengthy process of a legal name change and are still asked their "preferred name." No matter the intent, the impact is likely insulting to a person.
It could just be "name" and "legal name" and the legal name is not shown unless it is needed for something like billing.

For example, my name is Hannah, my nickname is Han and my legal name is the same as my name.

Hannah Baccouche: Agree 100% It's just "name" - my practice is 100% LGBTQ and I built my system from the ground up so that my "name" in the system is the name they use (e.g. the preferred name). The then added a custom demographic field (And mapped it) to be "Legal Name (if different)". This way all my chart templates and forms etc show both
