It would be nice to be able to switch at the patient and appointment level.

For example we are a massage clinic and see insurance patients. Some times they want the insurance covered massage and then self pay for another service that's not covered by insurance.

Agreed. I set up feature requests for separate cases on each individual. That would also allow for different payments (ie self pay vs Work Comp vs other insurance etc) and for different diagnoses to fall under each case. Please consider searching for that feature request and upvoting for that more robust change.
