Currently if we remove a diagnosis in a treatment note, it removes it from that specific note, but for the next visit it pops back in again. The current required workflow to prevent this is to go into the main chart, diagnosis section, and add an end date, which is quite obscure and difficult to remember.
If we're removing a diagnosis, we would like to not have it show up again next time in most instances. And the current workflow is resulting in duplications and denied claims when a provider accidentally leaves an old diagnosis on the note.
Could the functionality please be switched so that when we remove a diagnosis, it doesn't auto-populate on the subsequent notes? Or at the very least, that when we click remove it prompts us to ask if we want to remove it just from this note or all future notes (though honestly this won't typically be wanted, so is probably overkill).

I noticed this has rolled out yesterday. Unfortunately, it doesn't address our issue at all. In our pediatric outpatient practice, as an OT, I always have to manually remove the diagnosis codes that my speech therapists use and they remove mine. There are primary diagnosis that stay on all notes (like autism, cerebral palsy, etc). Now, as I've tested it, if archive a speech diagnosis, they no longer have the diagnosis available when they pull a note.
The suggestion to have the diagnosis codes pull forward by practitioner or ever note type is what we need. Thank you for all of your continued efforts!

I'd like to see something like this as we've been asking for this for years. BUT, if it's implemented in such a way that it applies to every note type, it could make our problems much worse.
We have both global diagnoses codes and diagnosis codes specific to each providers discipline. If I include speech therapy diagnosis on my notes (as an OT) some insurances won't pay for my time, especially if the client is also seen by a speech therapist on the same day.
I could only see this working for us if the dx's we remove on an OT note don't repopulate on our ongoing notes, with the same applying for our speech and physical therapists.

Somehow I have duplicate diagnosis codes with the dx spelled out differently for the same code. Would like to consolidate them. The code shows up twice on the superbill.
When I choose one dx, both show up (e.g., F90.2 Description-1 and F90.2 Description-2)
Note that the system doesn't allow me delete a duplicate code because they are both assigned to patients.

We’re working on a new feature: Diagnosis Archiving. Here’s how it will work:
When a diagnosis is archived, it will no longer be selectable in new Chart Notes, but it will remain visible in:
-Prior Chart Notes (for historical accuracy)
-The Patient Diagnosis tab (with a visual indicator showing it’s archived)
We’d appreciate your input on the following:
If you use the "Current Diagnosis" question on Chart Notes, would you prefer:
A) Archived diagnoses are automatically excluded from populating on new notes
B) Archived diagnoses still populate on new notes
When archiving a diagnosis, would it be helpful to see a list of all patients who currently have that diagnosis listed on their Patient Diagnosis tab?
Additionally, we’re working on disallowing edits to diagnosis codes to ensure historical notes remain accurate. Instead:
-You’ll be able to archive the old diagnosis
-And create a new one with the updated description or code

Archiving is about removing a diagnosis option for everyone (if I understand correctly), and that can be useful (for example we can't use F42. but we can use F42.9 with my insurers, so removing the former as an option is helpful).
BUT, that does not solve the issue in this post, which is about being able to a remove a specific diagnosis from a specific patient's notes going forward by making a change within the note itself (rather than having to navigate out to the diagnosis tab).

Tina Koro (IntakeQ) I was late getting on this thread. In our clinic we have up to 3 different disciplines. Often there is an overarching "primary" diagnosis (or 2 or 3) that should be on the notes of all three disciplines. The only way I could see archiving really working for us is if they were associated with a service categories. We have each of our appointment services broken down into general categories such as speech, OT, and PT. If the archiving feature allowed us to do this based on categories (and hence appointment types) so that speech diagnoses stayed out of OT and PT notes and vice versa, this would be ideal... Or perhaps the ability to adjust/control the "Current" diagnosis question a little more easily.
