Remove deactivated practitioners from "access control" list

When we assign our clients to their clinician via the "Access control" feature, we still see practitioners that have been disabled and are no longer working with us. There should be a way to remove them from this list (or have it automatically remove them once they have been deactivated). Thank you.

I concur. Along this line, it would be nice to be able to delete old service codes as well. We use service codes that are affiliated by practitioner, so when someone leaves we have to keep their codes in the system, otherwise they won't show up in reports.
