The questionairres that have been added, specifically the Cat-q are not in the format that they were published in. This invalidates the scoring. The questions must remain in the order that they were tested and validated in.

Please build the questianairre scoring so that each box on the table can be given it's own value so that all questionairres can be delivered as they are built.
