In many jurisdictions, practitioners, nurse, therapists and nurse are required to put their license number and credential/titles included with their signature. In fact some states - like in NJ, the name of the practitioner must be in every page.

I include this information in my e-signature box. You can set that up in the e-signature question "text" box of any form you want (see below). You can also add a practitioner name to the "footer" section of a note, so that it shows up on every page. Hope that helps!

Jennifer Hill; thank you for the tip. Great idea, but what about practices with multiple practitioners? I think that will only work great for single practitioner, but not in a group practice.
