Limit Provider Access to not see certain individual charts

We have a role that needs access to all charts (RN). Would like a way to remove her access to certain patients that she may know personally.

Another option could be a lock featured that you have to state why your accessing the chart. As my other office, for employees or relatives chart, anyone accessing them has to state why you went into the chart, if the person still needs to be able to access those chart for work related reasons.
