Goals Carry Over to Soap Notes

Right now on the patient file I have added personalized sections for long-term and short-term goals and added those to auto populate on my self-create soap note.
Is there a way for a SOAP note template to be created that auto-populates the goals? And then an option to "archive" goals and see the old versus new goals as each plan of care is updated?

Tina Koro (IntakeQ)I could see this being done similarly to the diagnosis question type. I typically use a matrix question type with two columns, with the first column being the goal and the second column being the data related to the goal so I can see them side by side. The problem with the matrix set up is that with pinned notes, data from prior notes will carry forward, so I have to start each session by making sure to erase the data column. My goals vary by client so using snippets in this situation would be a bit annoying. So a way to have set goals for a period of time that would autopopulate in notes and would have a way next to each goal to input data on that goal, including ways of doing numerical data that could be converted into a graph at possible, would be ideal. It would also be nice if there was a way to possibly input the baseline for the goal either through the note or through the profile (if it was set like the diagnosis question type) and then a "current" for when the goal ends so that we could see what goals the client has had and where they ended on them. Sometimes a goal is ended not because the goal was met but because the goal was too hard and needed to be discontinued until they had the skills needed to work on the goal.

We are looking into better goal management with a specific Goals question type. I'd love to get some feedback on this.
Can you confirm this is the workflow?
- Set SMART Goals: Therapist and client work together to define clear, measurable goals with a timeframe. These are entered into the visit note for the client.
- Choose Measures: Therapist picks tools (questionnaires, diaries) to track progress between sessions.
- Regularly Monitor: Progress is checked at sessions or through self-monitoring. Progress is recorded on the Visit Note.
- Evaluate & Adjust: Data is analyzed to see improvement. Goals or interventions may be adjusted based on progress. Notes are input via the Session notes.
Key Points:
Client-centered: Client is involved in the process.
Flexible: Adapts to individual needs.
Documented: Progress is tracked for future reference.

1.This seems right! But could the initial "visit note" the goals are entered in be in the plan of care that then transfers over to a SOAP note?
- The measures would be great for every soap (e.g. a percentage calculator, also great if it can carry over the percentage from last session to see comparison)
- An ability to generate a "report" that graphs and averages the data from the last few months would be great.
- This would be the next plan of care (every 6 months for us)
The transferring of goals I experienced with Theraplan
On SLPToolkit (subscription, used a lot in our school system) and Percentally (a one time paid app), you can generate report to graph and the show averages from a selected time frame.
SLPToolkit gives a free trial and Percentally is only about $10 if you guys are able to review how those softwares work

Tyson Edwards, I'm so happy to see this is planned. The screenshots below show my work flow, with Problem Situation, Goal, Measurable Objective, and Therapist Interventions defined in the Treatment Plan, with theObjectives and Interventions carrying over into the Progress Notes, with Radio Buttons to select if that objective was addressed or not in a particular appointment. However, the problem I have is that this is not flexible. If more than three goals/objectives are indicated, my templates don't work. Similarly, sometimes there is one Goal but multiple objectives under it. To have that flexibility would require a more complex Question Type with the flexibility to "add or remove rows" so to speak. Also, I feel like the Pinned Notes feature is not entirely bullet proof yet so I rely on Custom Fields hard mapped to each question that are defined in the Client Profile Settings, and hope that the new Goals question type would work with Custom mapped fields. In any case, much appreciated!!!

Tyson Edwards Another thought on Goals... In my world, and per insurance I work with (esp Medicaid behavioral health), Goals are primarily associated with an upfront Treatment Plan in addition to being tracked session to session. But it's the Treatment Plan that has start, revision, and end dates rather than each specific goal. Moreover, the Interventions are important as well. These fields might include an Intervention Description, Frequency, Responsible Provider, and Completed Date (optional).

Tyson Edwards We have 4 different disciplines in our clinic. So you would also need a way to differentiate between disciplines and choose which disciplines goals pulled into your treatment note form. Right now we too use the pinned note feature and link it to the correct form

Tyson EdwardsExcited to see this planned. I've attached some screen shots of what would be very helpful for tracking progress. Right now I am using another EHR to supplement PracticeQ so that I can have a "view data" option that will show the percentages for the SOAP notes, graph the percentages, generate an average, and then also has AI generate a progress report based on the data taken.

What we did was pin the goals in the treatment plan and in the SOAP Note - it's cumbersome because there is a ton of pinned information in each of the client's charts - but at least the goals do carry over. I don't know about archiving. We are just starting to use IQ/PQ, so not sure if it's going to work very well or not.

Hi, just a suggestion. You can create a snippet of goals and pull them into your note. When you are ready to use them, you just cut out the ones you don't want to use.

You have to make a .CSV file, one column only, and put all the goals you want in the column, one goal per row.
Email this file to IntakeQ and ask them nicely to turn it into a "autocomplete" data source. Bam Shazam! Make a question the autocomplete type on your SOAP note using the new data source.
