I would love the ability to create a custom rating scale. For example, not just a 1-5 or 1-10 rating, but the ability to also add images like smiley faces or colors.

Ideally, this would be the ability to have multiple pictures or numbers with a place for a description for each image, across a horizontal line. It would be even better if I could assign a number. to each area and a score be calculated and graphed over time. :)

Edited to add: I serve people across the lifespan, so the ability to customize a picture in relation to a rating would allow me to add smiley faces or characters for my younger clients.
