My referring partners are hoping for a printable page at the end of booking a patient so they can hand the printout over for the patient to take home.

Whether it's the confirmation page (shown in the screenshot) or the final page after confirmation, I'd love to be able to customize it. I'd add things like:

provider-specific fax number

a note that this appointment will be held via telehealth

a reminder to watch out for a booking link via the email provided (and even maybe a restatement of which email they provided)

This would only be possible if I could save a separate fax number to each of my providers. I'd like to be able to define things about my providers besides just their name and email address.
