When Uploading the Photo Id and Insurance Cards from The Registration Forms, only allow the client to upload vertical, so no manipulation is required during the upload process, therefore no error message.

Please add a feature to allow (or require) clients to upload a copy of their insurance card(s) and identification. I run a telehealth business and I need a way to compel the addition of these items!

Hi Kimberly - I created a spot on my intake form for them to upload an image of their insurance cards so I can confirm their coverage before the first appointment. I hope that is a helpful suggestion.

Kimberly Koessel Thanks for your patience - I took a little vacation and only just returned. Use the "File Attachment" option in your forms as a question. They can upload an image of their cards there. Alternatively, you can create a question that asks them to input their insurance information which you can then use to confirm their coverage by logging into your provider portal for that insurance company. Hope that helps!

Amber, we have it built into all of our consent forms. They have to attach a copy of their Driver's License. They can also upload labs, pics of their blood pressure, pulse, weight, etc... like if they are on a diet plan with us

Tyson Edwards: It is helpful that this is a feature in intake questionnaires, but is there also a way to include a file attachment field in consent forms? For example, my intake questionnaires are focused on obtaining client background, presenting concerns, history, etc., and then I attach various consent forms to the intake which reflect my policies and require signatures (for example, consent for services, telehealth consent, HIPAA acknowledgement forms, ROIs, etc.). I have a consent form for financial information/payment policies that asks for insurance information, credit card info, etc. I'd like to be able to ask clients to upload a copy of their insurance card to this form, but I don't see a way to do this. Can the file attachment field be added as a feature for consent forms, like it is for intake forms?

Tyson Edwards: doesnt work well when patients are using their ipads or if the patient did not fill out a form online and now are using our ipads in the office. just some code allowing camera access would be great

You can set this up as a file attachment. We created it in our Intake Form. They are required to upload a picture of the front and back of their insurance cards and their driver's license.
