Aetna and soon Cigna (oct 1st, 2025) are inappropriately downcoding E/M services 99214-15 codes to 99213. they are not denials but coming in as payments. Is there a way to mark or flag payments when a level of care has been changed by the payer. I have recently noticed this going through a chart review and noticed it has been going on since March 2025 with my aetna patients. They are not requesting records or asking for MDM information, just randomly downcoding and paying. So there must be a way to flag these changes in the system to help us catch this unlawful changes.
