Add Send Estimate (cost) option, and allow it to be converted into an invoice once patients decide to schedule the service/surgery/procedure.

It would be nice to be able to send an Estimate for surgery prior to converting the estimate to an invoice if the patient decides to schedule surgery. In my practice, I provide a quote for surgery during the initial consultation, based on their specific, individualized needs. Thank you for considering!
